A. Project & Broad Objectives

Lesotho is one of the 30 high TB burden countries in the world with an estimated annual incidence of 788 TB cases per every 100,000 population. Seventy four percent of TB cases are co-infected with HIV. TB treatment success rate has not gone beyond 74%, TB case fatality ratio is high and only a small proportion of TB re-treatment cases are offered testing for drug resistant TB bacilli. The mortality rate in TB/HIV co-infected TB patients is 223 per 100,000 population. Additionally, MDR-TB is on the rise with 3.2% and 6.8% of new and retreatment respectively TB cases are estimated to have MDR-TB. Despite Lesotho’s high burden of TB, TB/HIV and MDR-TB cases, the TB case detection rate is 49% which means more than half TB are not diagnosed and treated.

The Kingdom of Lesotho is among four countries participating in the Southern Africa Tuberculosis and Health Systems Support Project funded by the World Bank. This is a five year project with overall objectives to: (i) improve coverage and quality of TB control and occupational lung disease services in targeted geographic areas of the participating countries; and (ii) strengthen regional capacity to manage the burden of TB and occupational diseases. The project will target underserved populations with a high TB and/or TB/HIV burden, including mining communities, transport corridors and cross -border areas. Even though the main focus of the proposed project is on strengthening the health sector response to TB, there will be selective support for joint multi-sectoral activities to enhance overall impact. The proposed operation includes three components: (i) Innovative interventions supporting prevention, detection and treatment of TB and occupational health; (ii) Regional capacity strengthening for enhanced disease surveillance, diagnostics and management of TB and occupational lung diseases; and (iii) Learning, knowledge and innovation at national and regional levels. The project will target (i) mining and peri-mining communities; (ii) high TB burden “hot spots” areas; (iii) border districts with high levels of movement of people and goods; (iv) transport corridors; (v) labor sending areas and (vi) areas of high incidence of poverty in order to significantly impact the burden of TB in Lesotho

During preparation of the project, the Government of Lesotho prepared

Environmental and Social Management Framework (ESMF) for management of environmental and social impacts of civil works and
Infection Control and Waste Management Plan (ICWMP), to identify and address the environmental and social impacts associated with health services.
The Government is obligated to implement the project in full compliance with the requirements under both documents.

Objective of Assignment
The SATBHSS project is now hiring an Environment and Social (E&S) Specialist to coordinate and manage the implementation of the ESMF and ICWMP over the project lifetime.

C. Scope of Work

The responsibilities of the Environment and Social Specialist include, but not limited to, coordinating, monitoring and reporting on all aspects of the ESMF and ICWMP

Under the ESMF, these responsibilities include:

Conduct screening of project sites in accordance with ESMF\
Preparation of site-specific Environment and Social Management Plans (ESMPs)
Undertaking consultations of ESMPs with relevant stakeholders and maintaining documentation of such consultation
Getting the ESMPs approved by relevant authorities
isclosing all ESMPs publicly upon final approval
Ensuring the inclusion of ESMP requirements and Chance Find procedures into contractor documents
Undertaking regular field visits to all sites to supervise and monitor ESMP implementation during construction;
Preparing regular supervision and monitoring reports on implementation of ESMF, including field visit reports and photographic evidence
Reporting of ESMF compliance and ESMP implementation of all sites
Promote environmental and social enhancements, and better green designs, where possible through ongoing discussion
Documenting and reporting on any observed cases of any non-compliance with the ESMF and ESMP, ensure implementation of mitigation measures
Conduct regular meetings with Environmental Health and Ministry of Environment authorities and relevant stakeholders including communities to resolve issues and problems
Document good practice case studies regarding the Environment and Social Safeguards for the SATBHSS project
Commission an end-line independent environmental audit of ESMP implementation upon completion

Under the ICWMP, these responsibilities include

Evaluate the present ICWM practices and suggest improvements
Review present procurement plans for equipment and consumables needed for implementation of ICWM
Monitoring of ICWM practices during field visits
Identify gaps in laws and policies related to Infection control and waste management (ICWM)
Review technical guidelines for HCWM
Coordinate the setting up of institutional mechanism for the implementation and monitoring of ICWM at various levels
Evaluate the present ICWM training plan for health workers and commission update
Review and update the present training plan for rolling out training for healthcare workers and commission update
Evaluate the present training modules for health workers and commission update
Coordinate the implementation of training for all healthcare workers
Develop a public awareness strategy and implement the awareness activities
Maintain Coordination with other relevant ministries and agencies
Explore opportunities for private sector involvement in ICWM
Ensure budget line for continued ICWM is included in MOH budget in year 4, for continuation after project closure
Prepare Reporting Formats, Checklists and Guidelines, as necessary to facilitate monitoring and reporting
Undertaking regular field visits to healthcare centers to supervise and monitor ICWMP implementation;
Preparing regular supervision and monitoring reports on implementation of ICWMP, including field visit reports and photographic evidence
Reporting ICWMP implementation in healthcare centers, including completion of training, procurement of consumables etc.
Commission an independent assessment of ICWMP implementation in year 4 of project
Supervise

D.TERMS & CONDITIONS

The tenure of appointment shall be full-time for 2 year during implementation of civil works.
E.QUALIFICATION & EXPERIENCE

A Post Graduate in Environmental Sciences or Environmental Engineering;

Background in civil works/waste management and/or Health Care Waste Management is an asset

At least 8 years hands-on experience related to environmental and social project management

Experience working with the World Bank projects on Environment and Social Safeguards or other donor funded organization will be an added advantage

JOB TITLE Program Officer _ Childhood TB and Training coordination
PROJECT Southern Africa Tuberculosis and Health Systems Support Project
RESPONSIBLE TO National TB Manager

Job Summary
The Program Officer, Paediatric TB and Training is responsible to assist in planning, implementing and coordinating activities related to paediatric TB and for managing and coordinating all TB Program training activities. The incumbent will work under the supervision and guidance of the National Tuberculosis Program Manager.

Scope of Work
Coordinate all childhood TB related activities and ensure implementation of the TB guidelines on childhood TB diagnosis and management.
Work with NTLP staff and partners to develop appropriate strategies and plans to  manage childhood TB;
Work closely with the NTLP and partners and other stakeholders to identify program priorities, develop strategies and coordinating implementation of activities for strengthening and expanding of childhood TB case finding and prevention
Coordinate reporting on childhood TB in the MOH (case finding, diagnosis, patient management and outcomes)
Strengthen linkages with child health and ensure strengthening of TB detection and contact tracing by the child health department.
developing/updating child TB training curriculum in line with latest developments and newly approved NTP guidelines
Participate in discussions with NTLP and partners to identify training needs and coordinate development of training plan and training activities in Drug Susceptible TB including childhood TB, TB/HIV, MDR-TB for district level
Organise and coordinate orientation for newly deployed medical doctors and nurses on DS-TB including childhood TB, MDR-TB, TB/HIV
 Work in teams to develop TB annual work plan; identify and incorporate appropriate childhood TB related actions in work plan consistent with NTLP objectives and aligned with Programme needs;
Participate in workshops, seminars, task force and expert committees to identify policy, technical and programmatic gaps in increasing case notifications, diagnosis and treatment of childhood TB;
Support development of tools, job aids, Information, Education and Communication (IEC) materials as required for improving diagnosis and treatment of  Paediatric TB;
Monitor implementation of planned activities through periodic consultations, monthly progress review meetings, reports review, and field trips. Provide written feedback for improving performance and take corrective actions, as needed;
Work in  a team to prepare technical reports on training and ensure timely compilation of training information and produce quarterly and annual training reports as well as maintain data base of all trained individuals and by facility
Undertake supervision to districts and health facilities to monitor implementation of childhood TB interventions and review quality of data reported by implementing agencies
Support studies undertaken under the Southern Africa TB and Health Systems Project
Required Competencies
In addition to the above, the successful candidate is expected to demonstrate the following competencies:

Strong communication and facilitation skills and ability to establish good working relationships with colleagues and stakeholders in a sensitive environment. S/he must be able to respond quickly to requests for information
S/he must demonstrate strong interpersonal and motivational skills and sensitivity to the different environment as well as the ability to work with minimal supervision
Strong data analysis and interpretation. S/he must have ability to write clearly and concisely, and have sound quantitative  and qualitative skills
 Analytical and research skills are added advantage
Time management and ability to prioritise multiple tasks
Self-motivated person able to work without close supervision
Able to work effectively in a diverse team environment and travelling to different districts  independently for TB related activities

Qualifications

Qualifications and Experiences
A Nursing degree or any degree in a health-related field
A post graduate qualification in Paediatric Health Services including TB related field will be an added advantage
The candidate must have a recent training (@ least two years) on the clinical management of TB paediatric cases.
At least five years of relevant work (Paediatric TB and training in TB) experience in the field of TB and HIV control activities with at least two years in a supervisory role
Experience working in TB area including report writing and monitoring of district activities under any Ministry of Health Partner will be an added advantage
Computer literate, with high competency in using Microsoft office applications (Word, Excel,

A competitive salary will be offered as per Kingdom of Lesotho norms for public officers and World Bank guidelines
