With the high burden of TB and HIV, and the recent scale-up of HIV testing and treatment in South Africa, there is a great need to increase the number of health care workers that are able to manage these diseases safely and competently. Effective training coupled with follow up mentorship is extremely relevant to ensure capacity building at primary care level to deliver a high quality of care. MSF has created a “NIMART programme” (Nurse Initiated and Managed Antiretroviral Treatment Programme) with follow up on site mentorship to address the needs for decentralized adult, paediatric, and pregnancy ART care.

1, MAIN OBJECTIVES FOR POSITION
• To manage and report on the paediatric HIV clinical mentorship programme, with the aim to foster on going professional development to yield sustainable high-quality clinical care outcomes in Khayelitsha primary care clinics (PHCs) and midwife obstetric units (MOUs).
• To provide didactic lectures on paediatric HIV/TB, DR-TB, and mentorship during clinical meetings, MSF training courses, and provincial/institutional trainings.
• To develop, pilot, and disseminate training modules for Khayelitsha project’s innovative pilots (Youth medical package and Pre-exposure Prophylaxis –PreP)
• To be involved in operational research, including a pilot of HIV PreP .
  1. DUTIES AND RESPONSIBILITIES
    • Supervise the nurse mentorship team in the implementation of the decentralized clinical mentorship program for nurse mentees in the paediatric and DR-TB NIMART programme in Khayelitsha, in accordance with programmatic goals.
    • Provide clinical support for the nurse mentorship programmes, including standard treatment guidelines and clinical updates via NIMART clinical meetings.
    • Evaluate the outcomes of the paediatric NIMART programme, including the number of children initiated on ART by nurses, the total number of paediatric HIV initiations in the subdistrict, and potentially clinical outcomes such as virologic suppression
    • Provide HIV/TB/paediatric HIV mentorship to PHC nurses and medical officers as requested by provincial/municipal authorities or in cooperation with other MSF managers
    • Provide training for new nurse mentors and mentorship for new Department of Health mentorship programmes in Khayelitsha
    • Assist the Field Coordinator in advocacy for nurse initiated paediatric and DR-TB treatment (in future) at a provincial and national level
    • Manage the training and professional development requirements of the mentorship staff
    • Provide didactic trainings and lectures for MSF courses and community education
    • Attend continuing education programs to ensure current knowledge of clinical care, protocols, and guidelines
    • Participate in sub-district and provincial meetings to support the implementation of mentorship programmes in Western Cape
    • Pilot a study of PreP on different women subgroups through the development, implementation and outcome analysis plus dissemination.
    • Develop “Youth Clinical package” as part of a Youth Model of Care Pilot.
    • Carry out short detachments to regional MSF projects (dependent on need) to aid implementation of clinical nurse mentorship programmes and paediatric HIV trainings
The Clinical Mentor Manager will report to the Deputy Field Coordinator Medical (DFCM) of MSF Khayelitsha, along with on-going communication with others involved in teaching and mentoring. The clinical mentor will also need to communicate with facility managers and government partners on the implementation and outcomes of the mentorship programme and other pilot projects.
  1. EDUCATION AND EXPERIENCE:
Education:
• A tertiary medical qualification, preferably with a post-graduate qualification in HIV or infectious disease management and/or Publich Health.
• Current registration with the Health Professions Council of South Africa (HPCSA)
Strongly preferred experience and skills:
• South African citizenship or a valid work visa
• At least two years’ experience in managing HIV/TB care and treatment in adults and children in a lower socio-economic context, including the provision of ART and PMTCT (MDR-TB management skills will be consider as an extra-very valuable skill)
• Proven experience in and knowledge of clinical teaching and/or mentoring, with the ability to sit for mentorship accreditation upon hire
• Excellent organizational and computer skills including MS Word, Power Point, and Excel; ability to analyze and interpret data and write written reports
Required languages
• Fluent in English
• Ability to communicate in English and preferably Zulu, Xhosa, or other South African language
Personal characteristics:
• Excellent interpersonal skills with MSF staff, nursing staff, and patients, and ability to manage a team
• Ability to impart knowledge simply, clearly and appropriate to the mentee’s learning style
